Build a lead form and put it anywhere
Create a form in minutes, map answers to contact fields, then share the hosted page or embed it on any website with one script tag.
Lead Forms (the Forms entry in the sidebar) are standalone capture forms — no website builder required. Each submission can become a CRM contact, open a chat thread, and fire workflow automations.
Fields and mapping
Add fields (text, email, phone, textarea, dropdown, checkbox) and set each one’s mapping: an answer mapped to contact email or contact phone is what lets the platform deduplicate (a returning lead updates their existing contact instead of creating a duplicate) and what your workflows use to reach them. Map a textarea to contact notes to keep their message on the contact card. An answer whose field key matches one of your custom contact fields fills it automatically.
Settings that matter
Tag created contacts (e.g. india-lead) is how you target automations and bulk actions at this form’s leads. The success message is what submitters see — set expectations (“we’ll call within a few hours”).
Share it two ways
Publish, then either link to the hosted page (itzseo.com/f/your-form — great for WhatsApp bios, QR codes, social profiles) or embed it on any website by pasting the one-line script tag where you want the form to appear. The embed renders the same form inside your page and works on any platform — WordPress, Shopify, plain HTML.
Spam
A hidden honeypot field silently catches bots: their submissions are stored as spam and never create contacts, chats, or automation runs.
